Prosperity came in the 18th cent. with foreign commerce, and after the American Revolution, industrial development was rapid. The Brown brothers, John, Nicholas, and Moses, played leading roles in the growth of the town, prospering in foreign trade and fostering the textile and other industries. In 1842, Thomas W. Dorr led a rebellion that collapsed after an abortive assault on the armory there. The city became sole capital of Rhode Island in 1900 (Newport had been joint capital until then). In 1901 the state legislature began to meet in the impressive marble-domed capitol designed by McKim, Mead, and White. Providence is the seat of the noted Rhode Island School of Design (RISD), some of whose work is related to the city's famous silverware and jewelry industry, and of RISD's museum of art. It is also the site of Brown Univ., Johnson and Wales Univ., the New England Institute of Technology, Providence College, and Rhode Island C ollege. It has several noted libraries, including the John Carter Brown Library of Brown Univ. and the Atheneum (1753), one of the oldest libraries in the United States. Among the city's many historic structures are the old statehouse (where the general assembly met 1762â€"1900; now a courthouse), the old market building (1773), the Stephen Hopkins House (c.1755), the John Brown House (1786), and the First Baptist Meetinghouse (1775; the congregation was organized in 1638). The city has monuments to Oliver Hazard Perry (1928) and Nathanael Greene (1931). On Prospect Terrace is Leo Friedlander's heroic statue of Roger Williams (1939). Another memorial to the founder is in Roger Williams Park, which contains a museum of natural history and a natural amphitheater. The Capital Center District, where construction began in the early 1980s, and Waterplace Park have contributed to the city's downtown revival. My Personal Philosophy of Education When I was a little girl all I ever wanted to do was be a teacher. My neighbors and I would get out our little chalkboards and take turns teaching each other things that we had learned in school that day. We would spend hours in the basement grading tests or quizzes that we had made up ourselves. We had so much fun pretending to be teachers. As I got older I realized that a teaching salary may not be enough for me to get by on, so I decided to go to Marshall University and major in sports medicine. I have had may injuries due to sports over my lifetime and this seemed like a good option for a future career. The methods of teaching that I plan on using are lecture, recitation, and discussion. I also plan to use hands on experiments for the non-auditory learners in my class. I am going to include the teaching of tolerance in my classroom. I want my students to be accepting of all people. I do not want them do see differences as disabilities. They should see people for who they are on the inside, not judge them for their race, gender, or sexual preference. Discipline in the classroom is very important. My classroom is going to have a set of rules, compiled by the students and myself on the first day of class. If the students have a say in the rules they are more likely to follow them. After earning a Bachelors Degree in elementary and special education, I plan to start working right away. Either as a substitute or as a full time teacher. Additionally, there are the beginnings of evidence of a gay gene that codes for fetuses to be homosexual while in the mothers womb. Some scientists believe that homosexuality is linked to fecundity, which was later revealed to mean fertility (Adriaens and De Block). It is believed that matrilineal fecundity is highly linked to male homosexuality (Adriaens and De Block). It was never discussed further on who was involved in these studies, but there had to have been some compelling evidence. Because she was on the slope of the dune, the light reached her a few seconds before it touched the beach, and she watched it flow around her and along toward Will; and then she saw the hand-high figure of the Chevalier Tialys, standing by Will’s head, clear and wide awake and watching them. â€Å"The thing is,† said Lyra, â€Å"they can’t make us do what they want. They got to follow us. She put the alethiometer away and wandered back to Will, with Pantalaimon as large as he could make himself and lion-shaped, in the hope of daunting the Gallivespians. The man was using his lodestone apparatus, and when he’d finished, Lyra said: â€Å"You been talking to Lord Asriel?† â€Å"To his representative,† said Tialys. â€Å"We en’t going.† â€Å"That’s what I told him.† â€Å"What did he say?† â€Å"That was for my ears, not yours.† â€Å"Suit yourself,† she said. â€Å"Are you married to that lady?† â€Å"No. We are colleagues.† â€Å"Have you got any children?† â€Å"No.† Tialys continued to pack the lodestone resonator away, and as he did so, the Lady Salmakia woke up nearby, sitting up graceful and slow from the little hollow she’d made in the soft sand. The dragonflies were still asleep, tethered with cobweb-thin cord, their wings damp with dew. â€Å"Are there big people on your world, or are they all small like you?† Lyra said. â€Å"We know how to deal with big people,† Tialys replied, not very helpfully, and went to talk quietly to the Lady. They spoke too softly for Lyra to hear, but she enjoyed watching them sip dewdrops from the marram grass to refresh themselves. So maybe we better not mention it at all. We know where we’re going. So we’ll just go and not talk about it, and they’ll have to put up with it and come along.† â€Å"They can’t hear us now. Tialys, controlling himself, said, â€Å"We don’t accept one-sided demands. You must give something in exchange. You must let me have it when I want to send a message, but you will always know when that happens, and we shall not be able to use it without your agreement. That will be our guarantee. So here’s where we’re going: we’re going to the world of the dead. We don’t know where it is, but the knife’ll find it. If we went to him we’d have to do what he wants, and he wouldn’t think of Roger at all – that’s my friend who died – it wouldn’t matter to him. But it matters to me. To us. So that’s what we want to do.† â€Å"Child,† said Tialys, â€Å"when we die, everything is over. There is no other life. You have seen death. You’ve seen dead bodies, and you’ve seen what happens to a daemon when death comes. It vanishes. What else can there be to live on after that?† â€Å"We’re going to go and find out,† said Lyra. â€Å"And now we’ve told you, I’ll take your resonator lodestone.† She held out her hand, and leopard-Pantalaimon stood, tail swinging slowly, to reinforce her demand. The great insects were eager for flight, but the command of their riders was absolute, and Lyra, watching them in daylight for the first time, saw the extraordinary fineness of the gray silk reins, the silvery stirrups, the tiny saddles. Will took the knife, and a powerful temptation made him feel for the touch of his own world: he had the credit card still; he could buy familiar food; he could even telephone Mrs. Cooper and ask for news of his mother – The knife jarred with a sound like a nail being drawn along rough stone, and his heart nearly stopped. If he broke the blade again, it would be the end. After a few moments he tried again. There was no sound of human life, though a loud buzzing, so active and vigorous that it sounded like a machine, came from the stables. He’d just seen a man’s legs, one with a shoe on and one without, protruding from the thickest part of the bushes. The two spies were already at the farmhouse door, which was ajar. He found himself in a big square kitchen, an old-fashioned place with white china on a wooden dresser, and a scrubbed pine table, and a hearth where a black kettle stood cold. Next door there was a pantry, with two shelves full of apples that filled the whole room with fragrance. The silence was oppressive. Lyra said quietly, â€Å"Will, is this the world of the dead?† The same thought had occurred to him. But he said, â€Å"No, I don’t think so. It’s one we haven’t been in before. Look, we’ll load up with as much as we can carry. There’s sort of rye bread, that’ll be good – it’s light – and here’s some cheese†¦Ã¢â‚¬  When they had taken what they could carry, Will dropped a gold coin into the drawer in the big pine table. â€Å"Well?† said Lyra, seeing Tialys raise his eyebrows. â€Å"You should always pay for what you take.† At that moment Salmakia came in through the back door, landing her dragonfly on the table in a shimmer of electric blue. â€Å"There are men coming,† she said, â€Å"on foot, with weapons. They’re only a few minutes’ walk away. And there is a village burning beyond the fields.† And as she spoke, they could hear the sound of boots on gravel, and a voice issuing orders, and the jingle of metal. â€Å"Then we should go,† said Will. He felt in the air with the knifepoint. And at once he was aware of a new kind of sensation. The blade seemed to be sliding along a very smooth surface, like a mirror, and then it sank through slowly until he was able to cut. But it was resistant, like heavy cloth, and when he made an opening, he blinked with surprise and alarm: because the world he was opening into was the same in every detail as the one they were already standing in. â€Å"What’s happening?† said Lyra. The spies were looking through, puzzled. But it was more than puzzlement they felt. Just as the air had resisted the knife, so something in this opening resisted their going through. Will had to push against something invisible and then pull Lyra after him, and the Gallivespians could hardly make any headway at all. They had to perch the dragonflies on the children’s hands, and even then it was like pulling them against a pressure in the air; their filmy wings bent and twisted, and the little riders had to stroke their mounts’ heads and whisper to calm their fears. But after a few seconds of struggle, they were all through, and Will found the edge of the window (though it was impossible to see) and closed it, shutting the sound of the soldiers away in their own world. â€Å"Will,† said Lyra, and he turned to see that there was another figure in the kitchen with them. His heart jolted. It was the man he’d seen not ten minutes before, stark dead in the bushes with his throat cut. He was middle-aged, lean, with the look of a man who spent most of the time in the open air. But now he was looking almost crazed, or paralyzed, with shock. His eyes were so wide that the white showed all around the iris, and he was clutching the edge of the table with a trembling hand. His throat, Will was glad to see, was intact. He opened his mouth to speak, but no words came out. All he could do was point at Will and Lyra. Lyra said, â€Å"Excuse us for being in your house, but we had to escape from the men who were coming. I’m sorry if we startled you. I’m Lyra, and this is Will, and these are our friends, the Chevalier Tialys and the Lady Salmakia. They don’t look normal, exactly†¦ There was a man I used to see in my town, and he used to walk about outside the shops always holding the same old plastic bag, and he never spoke to anyone or went inside. Maybe my world’s full of ghosts and I never knew.† â€Å"I don’t think mine is,† said Lyra doubtfully. â€Å"Anyway, this must be the world of the dead. These people have just been killed – those soldiers must’ve done it – and here they are, and it’s just like the world they were alive in. I thought it’d be a lot different†¦Ã¢â‚¬  â€Å"Will, it’s fading,† she said. â€Å"Look!† She was clutching his arm. He stopped and looked around, and she was right. Not long before he had found the window in Oxford and stepped through into the other world of Citt? ¤gazze, there had been an eclipse of the sun, and like millions of others Will had stood outside at midday and watched as the bright daylight faded and dimmed until a sort of eerie twilight covered the houses, the trees, the park. What was happening now was like that, but odder, because the edges of things were losing their definition as well and becoming blurred. â€Å"It’s not like going blind, even,† said Lyra, frightened, â€Å"because it’s not that we can’t see things, it’s like the things themselves are fading†¦Ã¢â‚¬  The color was slowly seeping out of the world. A dim green gray for the bright green of the trees and the grass, a dim sand gray for the vivid yellow of a field of corn, a dim blood gray for the red bricks of a neat farmhouse†¦ The people themselves, closer now, had begun to notice, too, and were pointing and holding one another’s arms for reassurance. The only bright things in the whole landscape were the brilliant red-and-yellow and electric blue of the dragonflies, and their little riders, and Will and Lyra, and Pantalaimon, who was hovering kestrel-shaped close above. They were close to the first of the people now, and it was clear: they were all ghosts. Will and Lyra took a step toward each other, but there was nothing to fear, for the ghosts were far more afraid of them and were hanging back, unwilling to approach. Will called out, â€Å"Don’t be afraid. We’re not going to hurt you. Where are you going?† â€Å" They looked at the oldest man among them, as if he were their guide. â€Å"We’re going where all the others go,† he said. â€Å"Seems as if I know, but I can’t remember learning it. Seems as if it’s along the road. We’ll know it when we get there.† â€Å"Mama,† said a child, â€Å"why’s it getting dark in the daytime?† â€Å"Hush, dear, don’t fret,† the mother said. â€Å"Can’t make anything better by fretting. The Chevalier Tialys had spoken to Salmakia before skimming ahead, and Will and Lyra watched the dragonfly with eyes greedy for its brightness and vigor as it got smaller and smaller. The Lady flew down and perched her insect on Will’s hand. â€Å"The Chevalier has gone to see what’s ahead,† she said. â€Å"We think the landscape is fading because these people are forgetting it. The farther they go away from their homes, the darker it will get.† â€Å"But why d’you think they’re moving?† Lyra said. â€Å"If I was a ghost I’d want to stay in the places I knew, not wander along and get lost.† â€Å"They feel unhappy there,† Will said, guessing. â€Å"It’s where they’ve just died. They’re afraid of it.† â€Å"No, they’re pulled onward by something,† said the Lady. â€Å"Some instinct is drawing them down the road.† And indeed the ghosts were moving more purposefully now that they were out of sight of their own village. The sky was as dark as if a mighty storm were threatening, but there was none of the electric tension that comes ahead of a storm. The ghosts walked on steadily, and the road ran straight ahead across a landscape that was almost featureless. From time to time one of them would glance at Will or Lyra, or at the brilliant dragonfly and its rider, as if they were curious. Finally the oldest man said: â€Å"You, you boy and girl. You ain’t dead. You ain’t ghosts. What you coming along here for?† â€Å"We came through by accident,† Lyra told him before Will could speak. â€Å"I don’t know how it happened. We were trying to escape from those men, and we just seemed to find ourselves here.† â€Å"How will you know when you’ve got to the place where you’ve got to go?† said Will. â€Å"I expect we’ll be told,† said the ghost confidently. â€Å"They’ll separate out the sinners and the righteous, I dare say. It’s no good praying now. It’s too late for that. You should have done that when you were alive. No use now.† It was quite clear which group he expected to be in, and quite clear, too, that he thought it wouldn’t be a big one. The other ghosts heard him uneasily, but he was all the guidance they had, so they followed without arguing. And on they walked, trudging in silence under a sky that had finally darkened to a dull iron gray and remained there without getting any darker. The living ones found themselves looking to their left and right, above and below, for anything that was bright or lively or joyful, and they were always disappointed until a little spark appeared ahead and raced toward them through the air. It was the Chevalier, and Salmakia urged her dragonfly ahead to meet him, with a cry of pleasure. They conferred and sped back to the children. â€Å"There’s a town ahead,† said Tialys. â€Å"It looks like a refugee camp, but it’s obviously been there for centuries or more. And I think there’s a sea or a lake beyond it, but that’s covered in mist. I could hear the cries of birds. And there are hundreds of people arriving every minute, from every direction, people like these – ghosts†¦Ã¢â‚¬  The ghosts themselves listened as he spoke, though without much curiosity. They seemed to have settled into a dull trance, and Lyra wanted to shake them, to urge them to struggle and wake up and look around for a way out. â€Å"How are we going to help these people, Will?† she said. He couldn’t even guess. As they moved on, they could see a movement on the horizon to the left and right, and ahead of them a dirty-colored smoke was rising slowly to add its darkness to the dismal air. The movement was people, or ghosts: in lines or pairs or groups or alone, but all empty-handed, hundreds and thousands of men and women and children were drifting over the plain toward the source of the smoke. The ground was sloping downward now, and becoming more and more like a rubbish dump. The air was heavy and full of smoke, and of other smells besides: acrid chemicals, decaying vegetable matter, sewage. And the farther down they went, the worse it got. There was not a patch of clean soil in sight, and the only plants growing anywhere were rank weeds and coarse grayish grass. Ahead of them, above the water, was the mist. It rose like a cliff to merge with the gloomy sky, and from somewhere inside it came those bird cries that Tialys had referred to. “Story of an Hour” and “Desiree’s Baby” Essay Example

â€Å"Story of an Hour† and â€Å"Desiree’s Baby† Essay Kate Chopin lived a conventional life, marrying young to a wealthy businessman and from a prominent family herself.   She began writing to help support her family when she was widowed at a young age and left with 6 children to take care of and only a meager salary to live on.   She was almost an immediate success in the literary field, but almost stopped writing completely after the publication of The Awakening, which had themes of personal and sexual freedom for women and shocked her American audiences.   Despite that, her work is still considered to be a prominent figure in early feminist literature.In the time that Chopin was writing, and the time she was writing in, both women and African Americans were considered citizens of secondary class.   They did not have the same rights, freedoms, and privileges as white males.   Women were males’ subordinates, expected to live and breathe for their men and their men’s benefit.   Any woman that strayed from this male-servicing mentality was considered to be an aberrant female—not normal, unfeminine, anti-woman.   Blacks were slaves, and that was as simple as that.   They were not people, they were property, and for a white woman to have a black child, meaning that she already betrayed her family and race by having sex with a black man and out of wedlock (since they were not allowed to intermarry) was considered among the highest atrocities.   The white men could sleep with the black female slaves, however, because (a) the slaves were their property and (b) women existed to fulfill men’s needs.â€Å"Story of an Hour† is about a woman who is told that her husband has died in a train accident.   Her friends were worried that the news would cause her severe devastation, but the result was quite the opposite.   She experienced relief, and was anxiously anticipating her newfound freedom†¦until her husband walked through the front door, alive and well, causing h er the â€Å"severe devastation† and resulting in her death.   â€Å"Desiree’s Baby† is about a woman named Desiree whose recently-born child shows signs of undesired genetic makeup—the child appears to be of African American descent.   Desiree, who was adopted by a prominent family, is ostracized by her husband and thrown out of her home with the child, because he assumes it is she who is of â€Å"black blood† and disgraced him and his family’s good name.   We find out at the end of the story that it is not her at all, but him, and he finds out after he had already sent Desiree and the baby away.The main character is â€Å"Story of an Hour† is Louise Mallard.   Mrs. Mallard has a heart condition, but that is not all that appears to be â€Å"weak† about her.   She seems to be a very frail woman, the sort of woman who allows life to happen to her and who is a victim of her circumstance.   She is married to a man even she admits to be a wonderful person, and she never wanted for lack of love and affection.   Her situation appears to be a rather good one: loving husband who is not restrictive of her, well taken care of financially, friends and family that care for her tremendously.   And yet she still felt trapped.   She still felt as though she did not have her freedom, her ability to exist as a separate individual apart from her husband.   Her husband seemed to be the sort of man who would allow her to do whatever she wanted without question, yet she felt that the only way she could have her freedom was with him gone.   This mentality really only signifies her own mental weakness: that she is unable to take what resources she has to be independent and free and use them to her greatest advantage, instead once again being dependent upon another person for her own independence.Mrs. Mallard reacts in a number of ways when her husband dies.   When she first receives news of his death, sh e first weeps uncontrollably, then goes up to her room alone where she sits in silence, completely blank.   Then it dawns on her, and first she is gripped with terror, the excitement—she is free.   She became gripped with joy, until she saw her husband, still alive.   Whatever sorrow she felt initially was completely erased by the anxious anticipation of her freedom, her assertion of self.   She still felt a little sad for her dead husband, but that small feeling of sadness couldn’t compare to the immense joy she felt on top of it.Mr. Brently Mallard is a kind man.   He is, as described by Louise herself, a kind and loving man.   The way Louise herself describes him, it appears that he was a doting husband that would do anything for his wife and would also allow her to do anything she asked.   Their relationship seemed to be one that would be very positive; he did not appear to be a controlling man and would most likely have allowed Louise to pursue whate ver she wanted without question.I think that the end of this story shows Kate Chopin’s flair for tragic irony; that a woman with a heart condition would die of shock (and disappointment) upon seeing her husband still alive as opposed to finding out he died is incredibly ironic, and so much more considering her friends were reluctant to tell her for that very reason.   I think also that it is ironic that a woman with a husband who is so clearly understanding and loving would truly believe that her only chance of freedom is with him dead.   To me this shows a weakness in mind of the main character, and she deserved to be so overwhelmed by her husband’s reappearance that it killed her.   Louise Mallard would be a much more sympathetic character if her husband was cruel and abusive and controlling, but he was none of those things.   Therefore, her death, though definitely heavy in irony, is kind of anticlimactic.   By that point, I no longer cared what happened t o this fool of a woman.The character of Desiree in â€Å"Desiree’s Baby† is another victim of circumstance, but in her case it is completely outside of her control.   Desiree Aubigny is a beautiful young girl, who was abandoned at birth and taken in by an affluent Louisiana family.   She married what she thought to be a wonderful man, and soon gave birth to a son that she loved dearly.   Desiree was full of joy; she loved life fully, and loved her husband and son even more.   She was so blinded by love, in fact, that as people began whispering about the color of her son’s skin, she didn’t at first notice; and she never once saw the darkness in her son’s skin for herself.   It wasn’t until her husband Armand turned his back on her because of it that she began to pay attention to it.Desiree reacts in a variety of ways to the crisis in her life.   When the rumors begin, Desiree is still blissfully oblivious, and perfectly content wi th her child and her life.   Then she begins to sense that something is wrong, and this is when her husband starts to ignore her.   She is devastated by his treatment of her, but it still takes her some time to understand why.   When it finally dawns on her that her child is partially black, she is gripped with terror and confusion.   When her husband Armand accuses her ancestry as being at fault for their child’s color, she denies it, citing how white her skin is.   She cannot accept that she is to blame for what Armand is treating as an abomination.   In despair she writes her mother, who tells her to come home to her, and in one last pathetically hopeful attempt she questions Armand as to whether or not he wanted her to leave.   He did, and she left, dead inside, walking like a statue in a daze.Armand Aubigny is man in denial.   Poor adopted Desiree had no way of knowing that the man who would fall so madly in love with her would end up betraying her, turni ng his back on her, and all as a result of his own heritage (and which of her own she had no way of knowing, either, as a result of being abandoned by her birth parents).   Desiree is very much a victim in this case—a victim of a cruel, fickle man, who is more interested in protecting his own interests (such as his family’s name) than he is in standing by the love of his life’s side.   He turned his back on her, assuming immediately she was of African American descent, which she could not refute having been adopted, and cast her aside.   Armand is selfish, and his moods are like the weather—sometimes sunny and beautiful, other times stormy and vicious.   Before he and Desiree married, Armand had been a cruel and exacting man.   His nature became much gentler when he fell in love and married, and he was a wonderful, doting husband to Desiree.   But when his son’s heritage became increasingly more visible Armand’s cruelty came ragi ng back, and Desiree received the brunt of it.   He did not hit her—he just ignored her, as if she didn’t exist, which can be even more cruel.   And he remained in complete denial that he could possibly be at fault for the child’s blackness; and presumably continued to do so even after he discovered his own mother’s letter.The end of this story is another example of Chopin’s love of tragic irony, only in this story it was much more powerful.   Throughout the story the reader feels pity for this poor girl Desiree who had no way of controlling what was happening to her, no way to disprove what was being accused of her, and no way to talk sense into her boorish, pig-headed husband.   The entire story is heart-wrenching, and the reader experiences how badly Desiree suffers.   But at the end, when the audience discovers that it was Armand whose own mother was black, we realize how unjustly Desiree had been treated and just how cruel Armand rea lly was, and all because of him.   There is much more to sympathize with here, because Desiree’s suffering was much greater, and much less controllable.Louise Mallard and Desiree Aubigny are very similar characters.   Both women had husbands who, at least initially, adored them, and who were treated very well by their husbands.   Both women were victims of circumstances beyond their control—Louise with her heart condition and the false news, Desiree with her unknown parentage and black son.   Both women are dealt cruel blows at the hand of fate—Louise dies of shock at the sight of her living husband, and Desiree is thrown out of her home with her baby by her spiteful husband.   Both women are tragic characters.But for as many similarities exist between them, there are also a number of differences.   Desiree, for one, loved her husband and loved hr life.   She was happy and full of joy, until Armand turned against her.   Louise, on the other hand, was a miserable person who wished for death, and it was only at the thought of losing her husband that she felt excitement and joy.   Desiree did not want to lose her husband, and that was ultimately was drove her into despair.   Louise did want to lose her husband and was ecstatic when she thought she did; when she found out the contrary, it devastated her so badly it killed her.   Both are victims of circumstance, but Louise had circumstances she could change.   Whatever negative situation she believed herself to be in was of her own doing and existed primarily in her own head; her husband was not one who would try to control her or prevent her from doing anything she wanted to do, she just merely believed that to be true.   She was a victim of her own helplessness.   Desiree was in a negative situation completely outside of her control; she had no way of proving to Armand that she wasn’t black, since she had no way of knowing who her parents really were.   T hus her attempt at denial was futile, and she was a victim only of Armand’s cruelty.My opinion of â€Å"Story of an Hour† is that it allows the main character to be too much of a victim, without necessity.   If the readers are really supposed to feel sorry for her, then her husband probably shouldn’t be Mr. Wonderful.   â€Å"Desiree’s Baby† has a much more sympathetic main character in Desiree in that she was not able to prevent anything that happened to her from happening.   Everything that happened was as the result of other people’s doing; she had no role in any of it.   And yet she was the one who suffered the most and lost everything.   This is the ultimate cruelty.   Really, Louise Mallard simply just got what she wanted when all was said and done—before hearing about her husband’s death, she had been wishing for her own, which she got.  Ã‚   Desiree’s soul was crushed, and it was for no reason that she could control.

KEY ELEMENTS OF POSTPARTUM CARE: 6-12 hours 3- 6 days 6 weeks blood loss pain BP advice/ warning signs breast care temperature/infection lochia mood recovery anaemia contraception IMMEDIATE POST PARTUM CARE (After 3rd stage of labor till 2 hrs) Initial Postpartum Recovery Assessment: special attention to: Monitoring and assessment of maternal well being, prevention and detection of complications (e.g. bleeding, hypertension, and infections) Determine; Assess amount and the character of BLEEDING , Measure and document vital sign (BLOOD PRESSURE, pulse rate and temperature Other warning sign like fever, uterine involution, pain etc Status of the perineum, (tears laceration) Status of the fundus; position and firmness Document urine void within 6 hours. Ensure emptiness of urinary bladder Monitor for signs of bladder distension. After delivery IV fluids infusing type and amount Response of the woman and her partner to the newborn Give woman time with baby and family to facilitate bonding and celebrate the occasion. Status of the breasts once immediately after delivery and then again just before transfer to postpartum Allow women to rest Pain assessment; if the woman is experiencing any pain; Determine the characteristics, quality, timing, and relief after comfort measures, Investigate the source (e.g., after pains, episiotomy, painful urination, pain in the calves). If the woman had a cesarean delivery; check the incision dressing for intactness and determine incision bleeding if any complain for pain type, and success of analgesics and comfort measures to control the pain; toleration of ambulation status of the bladder; Provide hygiene care. Help woman to re dress Establishments of breastfeeding/ ROUTINE PROGRESSIVE POST PARTUM CARE (After 2hrs till 24 hrs) Take medical and pregnancy histories and perform physical examination that are important to alert for postpartum risk such as postpartum Hemorrhage, infection, and lead to normal healing First Visit HISTORY (should include significant pregnancy, labor and birth newborn history Review of Ante Partum chart and labs, problems which may need follow up Review of Intra Partum course and labs Check records: any complications during delivery Receiving any treatments Review of status since birth to postpartum period Review if patient is Tdap immunization status Obtains information about cultural factors influencing clients health and compliance Assesses clients and significant others feelings and level of understanding of sexuality, post partum. Observe the gravid woman for verbal and non-verbal clues to exclude post partum bluesdepression HIV status CONTINOUS POST PARTUM ASSESSMENT (ONCE PER SHIFT) Take the vital signs including of Respiratory characteristic s and Rule out labour breathing shortness of breath and chest pain. Regular Heart rate Height, And Weight Performs review of systems appropriate to POSTPARTUM Body parts Assessment and Probable findings Breasts Inspect the: Colostrum excretion breasts for signs of engorgement, nipples aversion/ redness, or cracks, then Palpate the breasts gently to determine if they are soft, filling, or engorged with milk Note if there is pain/ oedema/ swelling Abdomen Inspect the abdomen and note: striae, scars, shape and size of the abdomen Any organs enlargement and any masses. Palpate the abdomen to assess uterus involution (by height of fundas) determine consistency, tone, position, size/height in relation to the umbilicus Genitalia Inspect Vulva and perineum for: tear, swelling, pus. Observe external genitalia for color of skin, varicosities, and laceration, episiotomy stitches healing. vaginal opening for cystocele or rectocele. Vaginal discharge (lochia); special attention to color, amount and odor Bladder and bowel; Assess voiding amounts (more than 100 mL per each voiding) frequency If amounts smaller than 100 mL check for urinary retention, i.e. suprapubic distention Auscultate for bowel sounds in each quaderant. Ask the woman if she has had a bowel movement after delivery. Peripheral vascular Inspect the extremities for edema equality of pulses, and capillary refill. Check for Homans sign when the feet are dorsiflexed or woman is walking. Hemoglobin and hematocrit (hh). Compare the HH before delivery. Note the blood type and Rh. If the woman is Rh-negative, arrange for RhoGAM MEDICATION For pain relief advise: Topical cold therapy Paracetamol NSAIDs if not contraindicated In areas of high prevalence of iron deficiency anaemia, 400 mg ferrous sulphate (2 tablets) per day or once a week, with 250 ÃŽÂ ¼g folate for 4 months is recommended for pregnant and lactating women. In areas of low prevalence 1 tablet of ferrous sulphate daily may be sufficient. IMMUNIZATION Offer Anti-D immunoglobulin within 72 hours to every non-sensitized Rh-D-negative woman following the delivery of an RhD-positive baby. Complete TT vaccination for woman according to given schedule if required Offered an MMR (measles, mumps, rubella) vaccination following birth and before discharge from the maternity unit if they are in hospital if on antenatal screening women found to be sero-negative for rubella following the safety protocol Health Education for mother All women should be given information about the physiological process of recovery after birth, and that some health problems are common, with advice to report any health concerns to healthcare professional, in particular: Signs and symptoms of PPH: sudden and profuse blood loss or persistent increased blood loss; faintness; dizziness; palpitations/tachycardia. Signs and symptoms of infection: fever; shaking; abdominal pain and/or offensive vaginal loss. Signs and symptoms of thromboembolism: unilateral calf pain; redness or swelling of calves; shortness of breath or chest pain. Signs and symptoms of pre-eclampsia: headaches accompanied by one or more of the symptoms of visual disturbances, nausea, vomiting, feeling faint. Women who have had an epidural or spinal anesthesia should be advised to report any severe headache, particularly when sitting or standing Diet during post partum Caring breast while breast feeding Fundal Massage Perineum and Vaginal Care Pain Management: explain the non- medicinal ways of easing pain, such as applying warmth to the abdomen to help soothe after pains. Activities contribute to or prevent constipation. If non immune for rubella educate for rubella immunization SECOND VISIT (4-6 weeks) HISTORY Follow the guideline of ROUTINE POST PARTUM CARE (After 2hrs till 6hrs) CONTINOUS POST PARTUM ASSESSMENT follow the guideline of (After 2hrs till 6hrs) MEDICATION Follow the guideline of ROUTINE POST PARTUM CARE (After 2hrs till 6hrs) VACCINATION Follow the guideline of ROUTINE POST PARTUM CARE (After 2hrs till 6hrs) HEALTH EDUCATION Follow the guideline of ROUTINE POST PARTUM CARE (After 2hrs till 6 hrs) Postpartum Discharge Plan and complete all necessary assessment and care as per hospital policy by consider maternal and infant health and financial status. HISTORY Review of the womans physical, emotional and social well-being at taking in account the routine examinations Counsel mother family on baby care. ASSESSMENT OR EXAMINATION Follow the guideline of ROUTINE POST PARTUM CARE (After 2hrs till 6hrs) HEALTH EDUCATION Explain all take home medication with their effects and side effects. Allow them to resolve their queries. Educate mother about danger signs, personal hygiene, perineal care, postpartum exercises, follow-up visit baby care, baby mother immunization, breast feeding, Postpartum COMPLICATION MANAGEMENT essentc Hypertension during postnatal period Continue to assess patients for signs and symptoms or worsening of preeclampsia in the postpartum period.
